Ileum Intubation Made Easy

  • Lecture provides tips for intubating the terminal ileum during endoscopy.
  • Key positions for ileocecal valve intubation: 9 o’clock and 6 o’clock.
  • 9 o’clock position:
    • Locate ileocecal valve, pull scope back to cecal pole.
    • Engage valve by moving big wheel up and torqueing scope counterclockwise.
    • Use air and water to visualize ileum entrance, then gently push scope inside.
  • 6 o’clock position:
    • Start at cecal pole with valve below.
    • Move big wheel down to view valve lips and terminal ileum.
    • Torque scope right, apply air and water, push scope inside when ileum is visible.
  • Water is more effective from the left channel during 9 o’clock intubation; less critical for 6 o’clock.
  • Mastering ileum intubation improves skills for deep enteroscopy and overall endoscopic proficiency.
